Enrolment, Waitlist, and Enquiry Forms

Standard Digital Forms can be added to your OWNA Portal. This will allow for easy capture of data by families which can be controlled by Admin users within your centre. Each form will have a unique custom link (URL) which can be sent out via email or added to your website.

Price: $399 (includes all three forms)

The above templates will show your centre logo, name, and contact details as part of the setup (no additional costs), as well as any of the following changes/additions:

• Enrolment Form Sessions - 1 daily session (LDC) or two daily sessions (BSC & ASC)
• Enrolment Form Ts&Cs - provide a read-only PDF of your Terms and Conditions that we can attach to the form as a link
  
• Enrolment Form DDR Authorisation - parents can sign the DDR form when enrolling (Merchant Account Required)
• Waitlist or Enrolment Submission Fee - parents need to pay a fee to the centre in order to submit the form (Merchant Account Required)

Only the Enrolment Form can be customised:

Any changes required other than those listed above will incur an additional development fee starting from $300 (on top of the setup fee). The final price will be quoted based on the full extent of the changes required.

What can be changed:

• Additional Questions (in limited quantities)
• Change the wording of questions (so long as it doesn't make the form non-compliant with regulations)
• Making more fields mandatory (making too many fields required can make it harder for parents to enrol)

Not all customisations can be catered for (e.g. we cannot change the 'look and feel' or layout of the form).
