Submitting Custom Forms

Custom forms are digital forms built by service admins that can be made available to staff or parents to complete in the App.

📱 Menu > Forms

1. Select the form you want to complete
2. Answer all of the questions (some may be mandatory)
3. Press → to submit

💡Parents need to have Full Access or App Access permission for children linked to the form to submit the form!
