Overview

This section is where you can manage your custom form templates, as well as view responses from people who have submitted the form.

You can filter your forms by Active (Default), Draft, or Inactive, as well as by Staff Only or Parent Only.

Form Actions

Edit

Click the Edit button to make changes to the form questions or Deactivate the form (by clicking the red Deactivate button in the top right corner). You can also change the status of a published form to Drafts by clicking Save as Draft at the bottom of the page.

Duplicate

Duplicating a form will create an exact copy of your existing form. This is useful if you want to make a form with similar questions, make a staff/parent copy of an existing form, or test changes without changing the original.

Delete

āš ļø Deleting a form will permanently delete the form template and all responses to that form!

There is no way to reverse or undo this action. So it is important to be sure you want to delete a form before clicking the red x button.

Form Responses

Parent Forms

The responses page for parents form will summarise the children who the child has been signed for and the total number of responses at the top of the page. The names of the parents submitting the form and the date are also automatically captured at the top of each response.

Each response can be:

  • Printed using the blue Print button
  • Attach a copy to the child's documents by clicking Upload to Child's Documents
  • Deleted using the red x button (this cannot be undone)

Staff Forms

The responses page for Staff forms will count the total number of responses at the top of the page. The names of the staff submitting the form and the date are also captured automatically at the top of each response.

Each response can be:

  • Printed using the blue Print button
  • Attach a copy to the staff's documents by clicking Upload to Staff's Documents
  • Deleted using the red x button (this cannot be undone)
