Detailed Field Examples
Checkbox Group
A checkbox group allows multiple answers to be selected by the person submitting.
- Options ticked in the template will be ticked by default when the user goes to submit the form
⚠️ Make sure you enter the same values in the left and right columns!
Date Field
The date field allows the person submitting the form to select a date from the calendar (converted to YYYY-MM-DD format).
File Upload
Allows the person submitting the form to attach images or files from their mobile device.
Header
Separate different groups of questions with a custom header (e.g. Part 1: About Parent, Part 2: About Child, etc).
Number
Allows the person submitting to enter a number response.
Paragraph (static text)
Allows you to convey more information about the form to the person submitting it.
Radio Group
Allows only one option to be selected when submitting.

Select
Select functions the same as a Checkbox Group (allows multiple answers to be selected).

For a single line of text answer.
Text Area
For multiple lines of text to be entered as an answer.
